What to say in a written warning for employees?
own sports bar, our staff small difficult when calls in sick.
story:
waitress attempted call in sick on thursday (valentine's day) , informed not able cover shift because had chemotherapy treatment day , if in fact sick preform job need doctor's note due holiday. decided suck , go in, bartender said pouting few hours realized wasn't going home started working , didn't appear sick @ all. no coughing, no chills, no fever, no vomiting, said did not feel good.
friday afternoon text me telling me not coming work because still sick come in. not find cover shift , ended working alone on friday night (which impossible healthy person, less going through chemo.) turns out 1 of regulars saw @ dinner friends , husband, went facebook see , yep, there was. status tagging saying enjoying needed date night friends , family. livid!
told unacceptable call in sick go on date , if needed date night needed request off else has to. want make sure in written form , i'm not sure how word it.
infractions were: refusing provide documentation doctor upon request. not covering shift, , going on date leaving work place screwed. suggestions?
